ADULT PARTICIPANT WAIVER, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION & MEDIA CONSENT
By completing and submitting this form, I acknowledge that I am at least 18 years of age and agree to this waiver on my own behalf.
This agreement applies to all athletic activities, events, tournaments, camps, clinics, leagues, training sessions, competitions, and any other programming organized or hosted by Amity Sports LLC (collectively, the “Activities”).
1. ASSUMPTION OF RISK
I understand that participation in basketball and related athletic Activities involves inherent risks, including physical contact, falls, collisions, equipment issues, facility conditions, and actions or omissions of others.
These risks may result in injury including but not limited to sprains, strains, fractures, concussions, illness, or serious injury.
I voluntarily assume all risks associated with participation.
2. RELEASE OF LIABILITY
To the fullest extent permitted by law, I release and discharge Amity Sports LLC, its owners, staff, volunteers, officials, coaches, medical personnel, sponsors, vendors, and facility owners/operators from any and all claims arising from participation in Events, including claims of ordinary negligence.
3. MEDICAL AUTHORIZATION
In the event of injury or medical emergency, I authorize Amity Sports LLC staff and emergency personnel to provide or arrange medical treatment and transport as necessary.
I accept full responsibility for any resulting medical expenses.
4. MEDICAL INSURANCE ACKNOWLEDGMENT
I understand that Amity Sports LLC does not provide medical or accident insurance coverage. I am responsible for my own insurance coverage.
5. CONDUCT & REMOVAL
I agree to follow all event rules and understand that Amity Sports LLC may remove participants for unsafe, abusive, or unsportsmanlike behavior without refund.
6. MEDIA RELEASE
I grant permission for Amity Sports LLC to photograph, record, livestream, and use my image, likeness, and participation for promotional and marketing purposes without compensation.

UNIVERSAL YOUTH PARTICIPATION WAIVER, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION & MEDIA CONSENT
BY COMPLETING AND SUBMITTING THIS FORM, YOU ACKNOWLEDGE THAT YOU ARE THE PARENT OR LEGAL GUARDIAN OF THE MINOR PARTICIPANT IDENTIFIED BELOW AND AGREE TO THIS WAIVER ON BEHALF OF YOURSELF AND THE MINOR.
This agreement applies to all athletic activities, events, tournaments, camps, clinics, leagues, training sessions, competitions, and any other programming organized or hosted by Amity Sports LLC (collectively, the “Activities”).

2. RELEASE AND WAIVER OF LIABILITY
To the fullest extent permitted by law, I release, waive, and discharge Amity Sports LLC, its owners, officers, employees, contractors, volunteers, officials, coaches, medical personnel, event staff, sponsors, vendors, agents, representatives, and the owners, operators, managers, and lessors of any facility used for Events (collectively, the “Released Parties”) from any and all claims, liabilities, damages, actions, or causes of action arising out of or related to participation in Events, including claims based on ordinary negligence.
3. LIMITED SUPERVISION & EVENT ROLE
I understand and agree that Amity Sports LLC acts solely as an event organizer and host of scheduled athletic competition at third-party facilities.
Amity Sports LLC does not assume custody, guardianship, or general supervision of participants. Supervision is limited only to participants during scheduled Events occurring in designated playing areas during official event times.
I understand that Amity Sports LLC is not responsible for participants before check-in, after dismissal, or in parking areas, spectator areas, or any location outside of designated competition areas.
Responsibility for supervision, conduct, and safety outside of scheduled Events remains with parents, guardians, and/or team coaches.
4. CONCUSSION & INJURY ACKNOWLEDGMENT
I understand that basketball and related athletic activities carry a risk of head injuries, including concussions.

Symptoms may include headache, dizziness, confusion, nausea, or memory loss.
I understand that any participant suspected of having a concussion may be immediately removed from participation and may require medical clearance before returning to play.
5. MEDICAL AUTHORIZATION
In the event of injury or medical emergency, and if I cannot be reached, I authorize Amity Sports LLC staff, volunteers, officials, and emergency medical personnel to administer first aid, CPR, and obtain emergency medical treatment and transport as necessary.
I accept full financial responsibility for any medical care provided.
5. MEDICAL INSURANCE ACKNOWLEDGEMENT
I understand that Amity Sports LLC does not provide medical, accident, or health insurance coverage for participants.
I am responsible for maintaining appropriate insurance coverage for the minor participant.
7. CONDUCT & REMOVAL POLICY
I agree that the participant will follow all rules, facility policies, and event expectations.
Amity Sports LLC reserves the right to remove, suspend, or permanently prohibit participation by any athlete, coach, parent, or spectator whose behavior is deemed unsafe, disruptive, abusive, or unsportsmanlike, without refund.
8. MEDIA RELEASE
I grant permission to Amity Sports LLC to photograph, record, livestream, and otherwise capture the participant’s image, likeness, voice, and participation during Events.
I authorize Amity Sports LLC to use such media for event coverage, promotional content, website, social media, and marketing purposes without compensation or additional approval.
