AMITY SPORTS LLC – LIABILITY WAIVER AND RELEASE OF LIABILITY

BY COMPLETING AND SUBMITTING THE FORM BELOW, YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND VOLUNTARILY AGREE TO ALL OF THE FOLLOWING TERMS. YOU ARE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE AMITY SPORTS LLC AND ITS HOST VENUES.

1. General Assumption of Risk: I knowingly, freely, and voluntarily assume all risks, both known and unknown, associated with my participation in this sport, fitness, or recreational event hosted by Amity Sports LLC. I acknowledge that athletic activities involve inherent risks of physical injury. These risks include, but are not limited to: muscle strains, sprains, broken bones, concussions, lacerations, cardiac arrest, illness, permanent paralysis, or death. I certify that I am physically fit to participate and choose to do so voluntarily despite these risks.

2. Release and Waiver of Liability: I, for myself and on behalf of my heirs, assigns, and personal representatives, hereby release, waive, and forever discharge Amity Sports LLC, its owners, officers, managers, officials, agents, and volunteers, as well as the owners, operators, and landlords of the facility where this event takes place—specifically Amity School District 4J and the Amity School Campus (including the High School Gym, Middle School Gym, and Auxiliary Gym)—from any and all liability, claims, demands, or causes of action arising out of ordinary negligence, personal injury, illness, property damage, or wrongful death occurring during this event.

3. Head Injury & Health Protocol: I acknowledge that head injuries and concussions can occur in athletic environments. I agree that if I experience a severe blow to the head, display symptoms of a concussion, or feel physically unfit to continue, I will immediately remove myself from play and notify an event official.

4. Emergency Medical Treatment: In the event of an injury or medical emergency during this event, I hereby consent to receive first aid, CPR, or emergency medical treatment deemed necessary by on-site staff or emergency responders. I agree to be solely responsible for all financial costs associated with such medical care, ambulance transport, or hospitalization.

5. Rules and Conduct: I agree to abide by all facility rules, local regulations, and behavioral standards set by Amity Sports LLC. I understand that Amity Sports LLC reserves the right to immediately evict, suspend, or permanently ban any participant or spectator who displays aggressive, unsportsmanlike, dangerous, or disruptive behavior, without eligibility for a refund.

6. Media Release: I grant Amity Sports LLC permission to use photographs, video recordings, or digital media taken of me during this event for promotional, website, or social media marketing purposes.

AMITY SPORTS LLC – YOUTH PARTICIPANT LIABILITY WAIVER AND CONCUSSION ACKNOWLEDGMENT

BY COMPLETING AND SUBMITTING THE FORM BELOW, YOU CERTIFY THAT YOU ARE THE PARENT OR LEGAL GUARDIAN OF THE MINOR PARTICIPANT. YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND VOLUNTARILY AGREE TO ALL OF THE FOLLOWING TERMS, WAIVING CERTAIN LEGAL RIGHTS FOR YOURSELF AND THE MINOR TO SUE AMITY SPORTS LLC AND ITS HOST VENUES.

1. Parent/Guardian Consent & Risk Assumption: I give my full consent for the minor participant listed below to participate in sports, fitness, or recreational activities hosted by Amity Sports LLC. I acknowledge that athletic activities involve inherent risks of physical injury. These risks include, but are not limited to: muscle strains, sprains, broken bones, concussions, lacerations, cardiac arrest, illness, permanent paralysis, or death. I knowingly and voluntarily assume all risks on behalf of the minor.

2. Release and Waiver of Liability: I hereby release, waive, and forever discharge Amity Sports LLC, its owners, officers, managers, officials, agents, and volunteers, as well as the owners, operators, and landlords of the facility where this event takes place—specifically Amity School District 4J and the Amity School Campus (including the High School Gym, Middle School Gym, and Auxiliary Gym)—from any and all liability, claims, demands, or causes of action arising out of ordinary negligence, personal injury, illness, property damage, or wrongful death occurring to the minor during this event.

3. Oregon Concussion Compliance (Jenna's Law): I acknowledge that concussions are brain injuries caused by a blow to the head or body, and they can happen in any sport. I certify that I am aware of the signs and symptoms of a concussion (including headache, dizziness, confusion, or memory loss). I agree that if the minor participant exhibits any signs of a concussion during play, they will be immediately removed from the activity. Per Oregon law, the minor will not be allowed to return to play until they have been evaluated and received written medical clearance from a licensed healthcare professional.

4. Emergency Medical Authorization: In the event that the minor is injured or becomes ill during this event and I cannot be reached immediately, I hereby authorize Amity Sports LLC staff to secure first aid, CPR, ambulance transport, and emergency medical treatment from a licensed physician or hospital. I agree to assume full financial responsibility for all costs associated with such medical care.

5. Rules and Conduct: I agree that both I and the minor participant will abide by all facility rules, local regulations, and behavioral standards set by Amity Sports LLC. I understand that Amity Sports LLC reserves the right to immediately evict, suspend, or permanently ban any youth participant or spectator who displays aggressive, unsportsmanlike, dangerous, or disruptive behavior, without eligibility for a refund.

6. Media Release: I grant Amity Sports LLC permission to use photographs, video recordings, or digital media taken of the minor participant during this event for promotional, website, or social media marketing purposes.
